11. Richard Bracken, HCA Holdings

Total compensation: $46.4 million

Richard Bracken is chief executive officer of HCA Holdings, the largest for-profit U.S. hospital chain. His compensation package included a cash payment of $21.4 million for stock options that vested during the year. 

The Nashville, Tennessee-based company recently reported profit that missed analyst estimates. A slowdown in admissions growth and weakness in outpatient volumes have affected the company's performance.

Still, shares have risen 23 percent this year, giving the company a market value of $16.6 billion.
