Continuum Health Alliance (Continuum) and Kryptiq have formed a strategic relationship to deliver a comprehensive, turn-key solution for Chronic Care Management (CCM), enabling providers to confidently outsource CCM and participate in new value-based incentive and reimbursement programs. Continuum is uniquely positioned to provide clinical and non-clinical practice support on behalf of physicians given the company's legacy as a leading management service organization. When the Centers for Medicare and Medicaid (CMS) announced it would begin reimbursing Medicare physicians for comprehensive Chronic Care Management (CCM) services under CPT code 99490, it established a significant new revenue stream for providers. However, meeting CCM reimbursement requirements is often challenging. Providers must use a certified EHR, obtain and manage patient consent, deliver five core care management services, and provide at least 20 minutes of follow-up outside of the office (i.e., non-face time care) in conjunction with the CCM program. The combined offering leverages Kryptiq's CCM application, which, according to IDC Health Insights, is the industry's only complete technology solution for CCM. Continuum will bundle Kryptiq CCM in a services wrapper that includes telephonic coaching and call center support.
Kryptiq Appoints Joseph Siemienczuk as its Chief Medical Officer

Kryptiq announced the appointment of Joseph Siemienczuk, MD, as its chief medical officer. In this capacity, Dr. Siemienczuk will partner with Kryptiq's chief population health officer, Dr. Jacquelyn Hunt, to oversee the discovery, codification, and curation of the latest evidence based medical guidelines within Kryptiq CareManager, the company's population health management platform. Dr. Siemienczuk will also play an integral role in the development of value-based programs, as well as the corresponding clinical workflows housed within CareManager. Most recently, Dr. Siemienczuk led Providence Medical Group as its chief executive officer, where he had operational and financial accountability for more than 700 physicians and 80 clinics organized within Providence Health & Services' integrated delivery network.
