Premier Health and Humana Inc. have teamed up on a new value-based agreement that will focus on improving health and wellness for about 30,000 Medicare Advantage members in the Miami Valley. The value-based agreement reflects a shift away from the current episodic health care model to a value-based
model that emphasizes quality and patient care experiences. The accountable care approach aligns Premier
Health’s and Humana’s complimentary population health capabilities, including predictive analytics, chronic
disease management and wellness programs. Humana’s growing Medicare Advantage population throughout Southwest Ohio will have access to Premier Health’s leading network of integrated primary care and specialty physicians, hospitals, health centers, and outpatient centers at over 100 sites of service.
