PointRight, Inc. and Clinical Outcomes Management Systems Interactive Announces Strategic Partnership

PointRight Inc. and COMS Interactive announced that the two companies will leverage their collective clinical strengths and sales and marketing resources. PointRight and COMS will join forces to integrate predictive data analytics with real-time, bedside clinical care. In addition, the companies anticipate offering the COMS Daylight IQ disease management product suite in conjunction with PointRight Care Transitions rehospitalization services, which are part of PointRight's full predictive analytics suite. The focus of the combined products and services set is to further lower unnecessary hospital readmission rates throughout the continuum of care. PointRight Pro 30 rehospitalization, included in the PointRight Care Transitions family, is the industry-standard case-mix adjusted measure adopted by the American Health Care Association (AHCA) and incorporated into its Trend Tracker product. AHCA also recently submitted the measure to the National Quality Forum (NQF). The measure has also been validated by Brown University. PointRight's and COMS' joint commitment to reducing rehospitalization makes their partnership a natural fit in the industry. The COMS' product, Daylight IQ, has been successfully deployed in Skilled Nursing and Assisted Living Facilities and Home Care organizations throughout the US. The Software as a Service product features a series of integrated, disease-based library of clinical protocols that significantly empower the entire nursing team at the point of care, while improving communication between all parties. A key component of deploying Daylight IQ is the reduction in unnecessary hospital readmissions by up to 50%, a related decrease in premature mortality rates, and an increase in clinically successful discharges.

PointRight Inc. Announces Release of PointRight Pro 30

PointRight Inc. announced the release of PointRight Pro 30, an upgraded version of its popular tools leveraging big data to track and reduce rehospitalization rates in long-term care facilities. Formerly known as OnPoint 30, this new, industry-standard analytics service adds facility-level, case-mix adjusted rehospitalization rates for diagnostic sub-categories and for high, medium, and low-risk groups, capabilities previously only available as a separate product. Using one integrated analytics service, customers can now evaluate rehospitalization rates for common issues including congestive heart failure, COPD, stroke, diabetes, pneumonia and surgery. PointRight Pro 30 gives clients access to two important rates: observed and case-mix adjusted rehospitalization rates for both Medicare and all-payer groups, and compares these with national benchmarks. The service features root-cause analysis data correlated with average length of stay by diagnosis. It also provides the tools that facilities need to assess why residents returned to the hospital, determine areas of rehospitalization as well as possible preventative measures, and crosswalk rehospitalized resident with other outcomes to improve quality of care. Using graphical data display tools, i.e., 'smart downloads,' facilities can benchmark their progress by seeing how rates trend over time and can compare their rates by month to national averages. With penalties for excessive rates for acute-care partners reaching as high as 3% in 2014, PointRight Pro 30 allows providers to easily identify, trend and analyze rehospitalization cases to distill quality improvement opportunities, using web-based interactive analysis tools.

PointRight Inc. Announces Release of an Upgraded Version of its Professional Liability Snapshot

PointRight Inc. announced the release of an upgraded version of its Professional Liability (PL) Snapshot, a tool that accurately predicts the risk of liability claims for long-term care (LTC) facilities. PL Snapshot analyzes LTC facilities’ professional liability risks using proprietary predictive models built on a unique database of claims experience from over half of the industry facilities. These analytics support accurate risk analysis and targeting of risk management and enable claims to be evaluated more accurately and defended more effectively. The PL Snapshot tool provides on-demand decision support services for anyone who is a stakeholder in the LTC market, and is already used by insurance underwriters, real estate investment trusts (REITs) and merger and acquisition (M&A) organizations to assess an LTC operator’s risk before providing insurance coverage to them or investing in them. In Version 2, PL Snapshot now contains these enhanced features: A simple, color-coded report provides visibility on over 100 categories including staffing, care process levels, quality measures, and complaint history; A newly refined PL Risk score value evaluates a facility’s underlying risk of having a claim of over $250,000 compared to all other facilities across the nation, factoring in facility size and location, validated against PointRight’s extensive loss history database; A proprietary staffing-to-acuity analysis offers measured insights as to whether a facility is maintaining the specific staffing necessary to support its current level of acuity; Graphical trend analyses visualize how a facility’s quality and risk measures are tracking over time; and Reports are available on-demand via self-serve access from PointRight’s web-hosted Reports Portal.
