PointRight Inc. Announces Launch of its New Predictive Analytics and Performance Measurement Tools
Jun 3 15
PointRight announced the launch of its new predictive analytics and performance measurement tools. The toolset is ideal for those focused on improving consumer experience and clinical outcomes, including hospitals, Managed Care Organizations (MCOs), Accountable Care Organizations (ACOs), CMS Bundled Payment contractors and specialty care organizations. The new PointRight toolset has been developed based on over 20 years of experience in partnering with Skilled Nursing Facilities (SNFs). PointRight's analytics solutions enable payer organizations to improve the quality of post-acute networks, improve care transitions, track member's health status in real-time and ensure care plans address all of the members' risk factors. PointRight is increasing efficiency and decreasing the cost of care through four analytics-powered solutions: building and managing a quality network of skilled nursing facilities focused on improved patient results: by using nationally recognized quality metrics, clients are now able to identify the providers with the superior record of achieving the best patient outcomes. This transparency enables the forging of true partnerships. Now user can use the power of experience to set user skilled nursing facility network up for success. Understanding the member's needs, risks and impairment in real-time: Use existing post-acute data sets to accurately evaluate the needs of user members in a SNF. User can easily identify their impairment level for pain, mood, cognition and 'Activities of Daily Living' (ADLs) and proactively manage complications such as falls, pressure ulcers and re-hospitalizations. Matching members' needs to the right provider: assess a patient and match them to the SNF based on their individual needs and the provider's care capabilities. Not only do user improve the patient's outcome but user also help the SNF, giving them a patient for which they are best prepared and skilled. Identify Care Gaps and improve risk adjustment: Understanding a member's situation is critical to helping them to live to their fullest. Little is better at doing this than using the existing clinical records. CMS established standard records for post-acute care years ago. They are called the MDS (minimum data set) and the OASIS (Outcomes and Assessment Information Set). Now user can use these records to add to user existing care-gap and risk identification processes. Doing so will enable user to improve patient care by more fully understanding all the risks that need to be addressed when establishing a care plan.
PointRight Inc. Launches PointRight Five-Star FastTrack solution
Mar 2 15
PointRight Inc. announces the launch of its new market-tested, market-validated PointRight Five-Star FastTrack solution, which comes on the heels of the Centers for Medicare and Medicaid Services' (CMS) recent announcement that it is implementing changes to its Five-Star Quality Rating System. Developed with input from post-acute providers, the PointRight Five-Star FastTrack solution enables providers to use this solution to accurately determine how their facilities are performing in each of the CMS Five-Star Rating domains: Survey, Quality Measures and Staffing using the power of predictive analytics from PointRight and backed by its customer support team of seasoned clinicians and healthcare analytics experts. The PointRight Five-Star FastTrack tool offers the following solutions in one, easy-to-use service: Easily view current Five-Star Ratings. The most current ratings will appear as well as the Special Focus Facility candidate designation. Find the latest publicly available survey information. Providers can see their points for each survey cycle and the overall score for their facility. They can apply "What If" functionality to see how certain criteria or citations would impact their Survey and Overall ratings. Use an Expected Staffing Calculator by entering the number of residents that fall into each RUG category to calculate the staffing levels that would be anticipated based on the acuity of the residents in a facility. See current staffing as reported on your last survey. Providers can compare your staffing levels to national cut-points to see how close they are to the next Staffing star rating. They can apply ‘What If’ functionality to see how other staffing criteria would impact their Staffing Star. See how current Quality Measures are impacting Quality rating. Providers can compare their rating to the national cut-points to see how many points they need to get to the next-higher star rating. They can apply ‘What If’ functionality to see how making changes to the Quality Measure percentages would change their rating, and if they have been working on a QAPI project, they can enter their estimated impact to see how their work will pay off.
PointRight, Inc. and Clinical Outcomes Management Systems Interactive Announces Strategic Partnership
Oct 7 14
PointRight Inc. and COMS Interactive announced that the two companies will leverage their collective clinical strengths and sales and marketing resources. PointRight and COMS will join forces to integrate predictive data analytics with real-time, bedside clinical care. In addition, the companies anticipate offering the COMS Daylight IQ disease management product suite in conjunction with PointRight Care Transitions rehospitalization services, which are part of PointRight's full predictive analytics suite. The focus of the combined products and services set is to further lower unnecessary hospital readmission rates throughout the continuum of care. PointRight Pro 30 rehospitalization, included in the PointRight Care Transitions family, is the industry-standard case-mix adjusted measure adopted by the American Health Care Association (AHCA) and incorporated into its Trend Tracker product. AHCA also recently submitted the measure to the National Quality Forum (NQF). The measure has also been validated by Brown University. PointRight's and COMS' joint commitment to reducing rehospitalization makes their partnership a natural fit in the industry. The COMS' product, Daylight IQ, has been successfully deployed in Skilled Nursing and Assisted Living Facilities and Home Care organizations throughout the US. The Software as a Service product features a series of integrated, disease-based library of clinical protocols that significantly empower the entire nursing team at the point of care, while improving communication between all parties. A key component of deploying Daylight IQ is the reduction in unnecessary hospital readmissions by up to 50%, a related decrease in premature mortality rates, and an increase in clinically successful discharges.
