MatrixCare Announces the General Availability of its CareCommunity™ Population Care Management Platform

MatrixCare announced the General Availability of its CareCommunity™ population care management platform. CareCommunity—the truly integrated, spectrum-wide technology platform—is designed to revolutionize how LTPAC providers care for the U.S. senior population while also empowering them to successfully compete in an increasingly challenging healthcare environment. While long-term care residents do have acute episodes, 95% of their time is typically spent outside of an acute facility’s walls. Yet these LTPAC facilities typically have only a fraction of a senior’s full medical record, which makes it difficult to provide quality care. CareCommunity aggregates all pertinent clinical data from multiple entities involved in the care process—as well as from more than 500 consumer and commercial-grade telehealth devices such as Fitbits, wireless scales, and glucometers—which makes CareCommunity equally suitable for home health agencies providing services to seniors in their homes. By creating a truly integrated and fully connected health record, CareCommunity empowers seniors, their family members and other caregivers to optimize their health and well-being.
