Wellness and condition management programs are increasingly becoming a
critical component of employer benefit programs, but how can companies ensure
that the services are achieving the desired employee health and medical
financial savings outcomes?
Michael J. Connor, Dr.PH., senior vice president of customer analytics for
Alere Health, a leading provider of health management solutions, will
highlight what employers should know and understand about measurement and
reporting to ensure better care, cost savings and participant engagement
during the Care Continuum Alliance (CCA) Forum 2013 held October 23-25 in
Scottsdale, Ariz. Dr. Connor is also a member of the Health Enhancement
Research Organization (HERO) Think Tank, the HERO CCA Measurement Steering
Committee and the CCA Quality and Research Committee.
In his presentation at 3:30 p.m. on October 23, “The Value Cascade: Best
Practices for Reporting Program Value, Connor will cover the key measurement
domains and important population considerations that are crucial to the
success of population health management programs.
“The challenge has been how to measure and monitor the effectiveness of these
programs so that they can be adjusted to best meet client needs,” says Connor,
who leads customer analytics for Alere Health and has more than 30 years of
healthcare measurement and reporting experience. “At Alere, we have worked
with leading industry organizations to develop a value cascade, or framework,
that leads a client in a step-by-step process from initiation of a program to
patient engagement and adherence to the realization of performance goals.”

presentations at the CCA Forum:
*Dr. Connor, Rhonda Willingham, RN, BSN, executive vice president at Alere
Health, and Jerry Noyce, president and CEO of HERO, will present
“Communicating Value: A Strategic Business Imperative” at 8:00 a.m. on
Thursday, October 24.
*Craig Keyes, MD, CEO of Alere Health, will moderate a panel discussion
titled, “If You’ve Seen One ACO…” at 9:00 a.m. on Thursday, October 24.
*Dan Davis, R.Ph., CCP, director of product strategy for care gaps, will
present the poster “Clinical Gap Closure: Implications of Population
Differences on Success” in the poster exhibition area from 11:45 a.m. to
1:45 p.m. Thursday, October 24. The analysis looked at the implications of
population differences on the number and types of clinical gaps as well as
the success of gap closure for different segmented populations.
The annual CCA Forum brings together healthcare industry leaders, technology
