Conversio Health, a provider of integrated care management services focused on
treating patients with complex, chronic respiratory conditions, has received a
strategic investment from Heritage Group, a Nashville-based healthcare
investment firm. This investment expands the portfolio of companies backed by
the Heritage Healthcare Innovation Fund, which invests in businesses that
improve the quality, service and efficiency of healthcare delivery.

For patients with asthma or chronic obstructive pulmonary disease (COPD),
Conversio Health’s unique pharmacy-based model coordinates medications,
supplies and home delivery services. Conversio’s RITEMed™ program provides
patients with access to a unique team of clinical pharmacists, nurse care
managers, respiratory care practitioners, complex care coordinators and
nutritionists focused on each patient’s unique medical needs.
